Headwaters Charitable Trust is sponsoring a Passport to Fishing event. Proceeds of the event support US youth fly fishing projects in PA. The event allows those who register to fish anywhere in Pennsylvania waters that are open to the public without charge. The event will be from June 15th to the 22nd.

Participation Levels		RULES
Seniors Level: 16 years and older Youth Levels : 5- 7 yrs 8-10 yrs 11- 15 yrs Award levels: Most fish caught in one week (1,2 ,3 place) Most fish caught in one day (1,2, 3 place) Trout (1 each Rainbow, Brown, Brook in one day) (1,2.3 place Largest Fish caught (other than bass or trout) -need photo with catch card and measurement verifying caught during event dates. Largest Bass caught – need photo with catch card and measurement verifying caught during event dates. Largest Trout – need photo with catch card and measurement verifying caught during event dates. Youth : 5- 7 yrs (1,2,3 Place) 8-10 yrs (1,2,3 Place) 11- 15 yrs (1,2,3 place)		Must possess a valid PA fishing license. All fish must be caught in Pennsylvania waters open to the public without charge. Fish must be caught by legal methods. Measurements must be made with a metal ruler from tip of nose with mouth closed to tip of tail and record species and size. Scoring is on the honor system. For biggest fish must register with PA Fish and Boat Angler Awards Program, take photo with catch card and measurement. Catch and Release fish only. All score sheets must be signed and submitted via email or postal service by Saturday, June 29th^h. Winners will be announced no later than July 12^th. Any person found in violation of PA Fish and Boat Commission regulations or PA Department of Transportation regulations during event dates will be disqualified. Release form must me submitted with registration. Once registered a scoresheet and catch card will be emailed or mailed per person. Release form must also be signed and included with registration form.

I am aware that my/our participation in this challenge creates the risk of personal injury. My/our participation could also cause loss or damage to personal property and I expressly agree to assume all risks of personal injury/loss to personal property of other Challenge participants.
I release, waive, discharge, covenant not to sue Headwaters Charitable Trust or their volunteers, members, agents, employees, directors, officers, successors or assigns and agree to defend, indemnify and hold harmless (“Releasees”) from liability for personal injury, property damage, wrongful death or other loss, damage or cost, including reasonable attorney’s fees, that may incur or arise out of my use of participation in the Challenges for any and all loss or damage and any claims, actions or causes of action, whether known or unknown, arising out of or related in any way to my participating in the Challenge,.
The Agreement is intended to be as broad and inclusive as permitted by the laws of the Commonwealth of Pennsylvania and that if any portion is held invalid by a court of competent jurisdiction, the balance shall continue in full legal force and effect.
